NJSD VITALITY CHECK SCREENINGS
Know Your Numbers
Prevea on-site provides confidential biometric screenings at all NJSD locations in January and February each year. Screenings done outside of this timeframe will not count toward the incentive to receive the wellness premium rate. Current employees should schedule at their home-based building to ensure they have fob access. For security purposes, spouses and retirees will schedule the appointment on a Saturday at NHS or at the Health and Wellness Center. Be sure to read the Biometric Screening Instructions completely before scheduling any appointments.

If you have not yet created a Vitality account, please review the registration document linked here. Remember that Biometric Screenings and Vitality Health Reviews cannot be done prior to
January 1 each year.
INCLEMENT WEATHER
As you know, Wisconsin winter can be unpredictable. For the safety of our OHNs who are traveling from Green Bay and for our staff who travel from varied distances, screenings may be canceled if the weather is deemed to be unsafe for travel. Please review the policy before signing up.
VITALITY CHECK VERIFICATION FORM
If you are completing your biometric screening with a Primary Care Provider, this form must be completed at the time of your screening and uploaded to the Vitality website no later than February 14. In addition, you will need to manually enter the data listed on this sheet to the Vitality website in order to receive points for your Vitality Check. Without this step, it will show that your biometrics are incomplete.
PRENATAL VERIFICATION
Congratulations on your pregnancy! While you are pregnant, your biometric results may be out-of-range. Submit the Prenatal Care Waiver to Vitality along with a letter from your physician stating that you are pregnant and receiving prenatal care, and Vitality will accommodate you. Follow the instructions at the top of the form for submission.
BIOMETRICS PHYSICIAN'S WAIVER
If your blood glucose, blood pressure, BMI, total cholesterol or LDL measures are out of range and you are unable to meet the Reasonable Alternative Standard (RAS) recommended to you by Vitality or your employer, you may submit a Biometrics Physician Waiver. Vitality will then accommodate you by awarding in-range points for the biometric measures specified in your waiver.
VITALITY HEALTH REVIEW
Confidential Online Health Assessment
Completing the Biometric Screening is step 1. Step 2 is to log into the Vitality website or use the Vitality Today smartphone app to complete the Vitality Health Review. Employees, spouses, and retirees on the plan must each complete this assessment. It can be done from anywhere and takes less than 10 minutes to complete. You must complete this assessment in addition to your biometric screening in order to receive the wellness premium rate.
